At HomeCare Australia, we’re committed to providing personalised, high-quality support that meets your unique needs. A key tool in achieving this is your Care Plan—a document designed to guide your care and ensure it evolves with you.
Here’s why your Care Plan is so important and how it helps us deliver the best possible support.
What is a Care Plan?
Your Care Plan is a personalised document that serves as a roadmap for your care. It:
Lists your needs and challenges so we can identify areas where support is required.
Highlights your goals to help us work together to achieve them.
Outlines specific actions for you, your Care Worker, and your Care Manager to ensure everyone is on the same page.
By capturing your needs and goals in one place, your Care Plan helps us track progress and tailor our services to meet your priorities.

Why Keeping Your Care Plan Updated Matters
Life is full of changes, and your care needs can evolve over time. Regularly reviewing and updating your Care Plan ensures it stays relevant and aligned with your current circumstances.
Our Care Managers will contact you each month to:
Gather important clinical data, such as recent GP visits or health changes.
Update your Care Plan to reflect any changes in your health, goals, or needs.
This document also serves as essential evidence for My Aged Care to justify reassessments when needed.
By keeping your Care Plan accurate and up to date, we ensure your care remains focused on
your priorities and adapts to any changes in your life.
Need a Copy of Your Care Plan?
If you’d like a copy of your current Care Plan, let your care manager know. We’re happy to email it to you so you can stay informed and actively involved in your care journey.
