10-20 HOURS PER WEEK with potential to move to full-time

SALARY $65K - 85K pro rata + Superannuation (commensurate with experience)

You will have the opportunity to work directly with the CEO, external providers and engage in personalised coaching with leaders in the industry.

Following a recent restructure the opportunity has opened up for a high calibre individual to join our care management team part-time.


 

The Role 
 

This is an opportunity for an experienced Home Care Manager to work from home part time. 

 

As a Care Manager you will responsible for:- 

  • Aged Care Care Management and planning

  • NDIS Care Management oversight

  • Risk and Vulnerability assessments

  • Care plan budget oversight and management

  • Coordination of care plan service delivery and rostering

  • Client communication

  • Collation of clinical reporting data

  • Project delivery as required

 

This is a role where your strong customer service focus will shine and you will lead your team in effective problem solving, client retention strategies with exemplary clinical reporting, communication and management skills.  

 

You will be able to demonstrate evidence of a high degree of drive coupled with a passion for helping our most vulnerable citizens. You are committed to providing quality care within the community


 

Qualifications - Education or Professional Experience
 

  • Demonstrated experience and knowledge of the needs of older people, people diagnosed with dementia as well as those with a disability

  • Diploma in Community Services or relevant transferable experience

  • Experience with cloud based databases such as Visual Care or similar, Microsoft Office and Google drive and ability to learn new systems

  • An appropriate work space and quality internet such as high performing NBN or similar.

  • Current driver's licence

  • Federal Police Check

  • Current NDIS worker screening
